- Here's Iowa's judicial site with downloadable forms, but I don't see a form with that exact name: http://www.judicial.state.ia.us/Court_Rules_and_Forms/Small_Claims_Forms/
- There is no such thing. A lawsuit begins with a complaint, and except in small claims court a complaint requires much more than filling out some form. It requires a recitation of the facts establishing the personal and subject matter jurisdiction of the court as well as the factual basis for the suit, and a claim for damages supported by factual assertions. Unless you know what all that means, hire a lawyer.
- (1) Go to your court house and ask for one (but you will pay). (2) Go to a law library (find this out) and ask where you can get the forms. You may still have to pay something. But then at least you can prepare it yourself.
- Schaz gave you a great link, but I don't think you are going to find anything by that name. In most states, you file either a "Complaint" or a "Petition" to get a lawsuit started. Your description makes it sound like a document asking permission to file a lawsuit, which would be very unusual and not a form. However, the small claims form to sue for money is called "Original Notice - Action for Money Judgment". That might be what you are looking for?
